MQ-1B Predator accident report released Published April 2, 2009 Air Combat Command Public Affairs LANGLEY AIR FORCE BASE, Va (ACCNS) -- Pilot error caused the crash of an MQ-1B Predator at Kandahar Air Base, Afghanistan, November 2, 2008, according to an Air Combat Command Accident Investigation Board report released today. The $4 million Predator, assigned to the 62nd Reconnaissance Squadron, 451st Air Expeditionary Group, was destroyed on impact. There were no injuries or damage to other property or equipment. According to the report, the pilot commanded an aggressive turn to avoid entering a no-fly zone, which caused the aircraft to spin out of control and crash. Additionally, a heavy left wing due to carrying a hellfire missile, a significant crosswind, the autopilot not limiting the turn to 40 degrees of bank, and the ease of over-controlling Predator aircraft all substantially contributed to the mishap.
